Objective: The aim of the present study on small-sized deep interproximal cavities of maxillary premolars is to compare the sealing ability of a one-step self-etching adhesive, with or without selective enamel etching, and with an etch-and-rinse three step system. Methods: Standardized mesio-occlusal Class II cavities were prepared on 30 extracted maxillary premolars, with the cervical margin of the proximal box 1 mm below the cemento-enamel junction. The teeth were randomly assigned to the etch-and-rinse group (Total Etch, Ivoclar Vivadent; Adper Scotchbond, 3M ESPE) (G1, n=10), to the self-etch group (Scotchbond Universal, 3M ESPE) (G2, n=10) or to the self-etch group with selective enamel etching (G3, n=10). A centripetal open sandwich restoration technique was adopted in all groups. Adaptation at the cervical margin was assessed by SEM analysis of positive resin replicas of the specimens. After thermocycling (10000 cycles, 5°C/55°C) the microleakage at the cervical margin was determined by blue methylene penetration and stereomicroscope observation according to the scale adopted by Fabianelli et al. (2010). Specimens were longitudinally cut three times and leakage evaluated. Non-parametric statistical analysis was used to assess differences among groups (α=0.05). Results: All groups exhibited some degree of marginal leakage. The lowest dye penetration scores were registered in G2; the microleakage was found to be significantly higher in G1 and G3 (p<0.05), which performed similarly. SEM observation revealed satisfactory marginal adaptation and absence of voids in all groups, tough sporadic slight overhangs were detected. Conclusion: Under the conditions of the present study, the best sealing ability at the cervical level of small proximal cavities with the cervical margin 1 mm below the cemento-enamel junction was surprisingly achieved with a one-step system. The difficult management of the etchant gel in these cavities might hinder the adhesive procedure.
