Two cases of extracorporeal repair of severe lesions involving the distal branches of the renal artery are extensively described and discussed. Microsurgical techniques allow the perfect patency of the anastomoses of vessels of 1 mm or less in diameter. Work bench surge provides the best conditions for the use of the operating microscope. Renal artery reconstruction was accomplished in these two cases by using autogenous vascular grafts, arterial or venous.
