Aim of this study is to perform a perspective randomized controlled trial to verify if a microsurgery with magnification could improve the outcome of total thyroidectomy. The patients were consecutively and randomply allocated in two groups: A, surgery with magnification; B, surgery with no magnification. Paralysis of the recurrent laryngeal nerves and hypoparathyroidism were considered as the commonest compliactions. Mortality, morbility and operation time were calculated. The microsurgical approach with magnification, significatively reduced the operation time and improved the outcome of total thyroidectomy
