Clinical history and physical examination do not always allow a firm differential among the different possible cause of acute scrotum, and imaging is needed to provide a diagnosis. Differentiating torsion from infection is a clinical emergency. Inflammatory conditions of the scrotum are the most frequent cause of acute scrotal pain in the adult population. The acute scrotum has several less common causes. This article outlines the ultrasonographic and Doppler findings in patients with acute scrotum of nontraumatic origin.
