Paediatric patient safety is a clinical priority, although research specifically focused on de-velopmental age remains fragmented. This article analyses medical error-defined as the failure of a planned action or the adoption of an inadequate plan-through the examina-tion of five clinical cases. The analysis reveals how cognitive biases, ineffective communi-cation, the underestimation of parental concerns and the fragmentation of responsibility are critical determinants in the diagnostic-therapeutic process. This work emphasises that, alongside technical competence, it is essential to promote an organisational culture based on a critical analysis and a multidisciplinary approach. The sharing of adverse experien-ces emerges as a fundamental tool for reducing the recurrence of errors and ensuring ex-cellence in paediatric care.
